Temporary hair reduction is defined as a delay in hair growth, which usually lasts 1-3 months, consistent with the induction of telogen. Permanent hair reduction refers to a significant reduction in the number of terminal hairs after a given treatment, which is stable for a period of time longer than the complete growth cycle of hair follicles at the given body site.

Demand for a rapid, noninvasive method for hair removal has resulted in the introduction of various light sources for hair removal. Light source of hair removal commonly focuses on ruby, alexandrite, diode, and Nd:YAG lasers and intense pulsed light sources. These devices are designed to target either an endogenous chromophore (melanin) or an exogenous chromophore (carbon suspension, photosensitizer, and exogenous dye). Laser hair removal is one of the most recent systems of hair removal that has been introduced. Today, a large number of specific laser systems are available in the market, which makes the customer a bit apprehensive about their attributes, selection, treatment protocols and effects. Laser hair removal is one of the light sources of hair removal. Hair follicles are destroyed by light in a number of ways such as thermal, photomechanical or photochemical mechanism with generation of toxic mediators like singlet oxygen or free radicals.

Thermal mechanism for hair removal has recently been introduced to induce selective damage to hair follicles. Based on the principles of selective photothermolysis, this principle predicts that selective thermal damage of a pigmented target structure will result when sufficient quantity of light at a given wavelength is absorbed by the target in a time equal to or less than the thermal relaxation time of the target.

Melanin is the natural chromophore for targeting hair follicles in the visible to near-infrared region, Lasers or light sources that operate in the red or near-infrared wavelength region lie in an optical window of the spectrum in which selective absorption by melanin is combined with deep penetration into the dermis. Therefore, deep and selective heating of the hair shaft, the hair follicle epithelium, and the heavily pigmented matrix is possible in the 600nm to 1100nm region. However, melanin in the epidermis presents a competing site for absorption. Selective cooling of the epidermis has been shown to minimize epidermal injury.

Photomechanical destruction of hair has been attempted with very short nanosecond pulses. It has been done by the help of Q-switched 1064-nm Nd:YAG lasers, with and without carbon suspension. Short pulses are used to target hair follicles resulting in rapid heating of the chromophore (melanin). It leads to photo acoustic shock waves that cause focal photomechanical disruption of the melanocytes but not complete follicular disruption. Therefore, the Q-switched Nd:YAG lasers are not likely to produce long-term hair removal.

A distinction needs to be made between permanent and complete hair loss. Immediately after laser treatment, the hair shaft shows fragmentation with focal rupture into the follicular epithelium and thermal damage to the surrounding follicular epithelium. The extent of thermal damage is dependent on the pulse width but retains confinement on the spatial scale of the follicle itself. Histological changes are almost guaranteed in the person undergoing laser hair removal. However, such form of hair removal should be carried out under the supervision of a qualified dermatologist as it involves considerable degree of expertise.

Some people grow tired of shaving, waxing and plucking, and consider permanent hair removal as a logical course of action. Knowing what's available and how much it costs is a determining factor, as are method and convenience. Does permanent hair removal really work? Will hairs eventually grow back? Permanent hair removal can be accomplished with several different methods, but the most common is electrolysis. This method utilizes a light ray that can travel beneath skin layers, without damaging them, to pinpoint a hair follicle. Permanent hair removal is a misnomer, however. Heat generated in this light ray then damages or destroys the hair follicle, resulting in little or no hair growth for anywhere from three to six months. Eventually, the hair will grow back. Still, this is a viable option for many to facilitate active lifestyles and temperaments. To achieve this state however, several sessions are required, and each treatment is very expensive, averaging around $400 a visit.

Most people in the permanent hair removal industry define 'permanent' as being able to go months without having to shave again. Permanent hair removal using laser methods does achieve long-term hair removal but not permanent hair removal. Other methods of permanent hair removal with less satisfactory results are the use of electric tweezers, dietary supplements and non-prescription medications. Permanent hair removal is less likely with the use of tweezing, while half of people who tried a topical ointment achieved some degree of permanent hair removal, or at least to the degree that shaving was not required for at least a few weeks.

Some studies have shown that many people are unable to achieve permanent hair removal for various reasons, whether genetic or otherwise. Again, the new hair will not have any pigment and the laser will not be able to target it. This new hair will shortly enter the ANAGEN stage, and if it is exposed to a Laser treatment when it has entered the ANAGEN stage, then permanency can be expected.

In summary, each hair on your body will naturally be at a different part of the growth cycle, and that is why it will be necessary in most cases to undergo a number of Laser hair Removal treatments at varying intervals to completely destroy all hair follicles in the region you are targeting. Light source of hair removal commonly focuses on ruby, alexandrite, diode, and Nd:YAG lasers and intense pulsed light sources. These devices are designed to target either an endogenous chromophore (melanin) or an exogenous chromophore (carbon suspension, photosensitizer, and exogenous dye). Laser hair removal is one of the most recent systems of hair removal that has been introduced. Today, a large number of specific laser systems are available in the market, which makes the customer a bit apprehensive about their attributes, selection, treatment protocols and effects. Laser hair removal is one of the light sources of hair removal. Hair follicles are destroyed by light in a number of ways such as thermal, photomechanical or photochemical mechanism with generation of toxic mediators like singlet oxygen or free radicals.

Therefore, deep and selective heating of the hair shaft, the hair follicle epithelium, and the heavily pigmented matrix is possible in the 600nm to 1100nm region. However, melanin in the epidermis presents a competing site for absorption. Selective cooling of the epidermis has been shown to minimize epidermal injury.

Photomechanical destruction of hair has been attempted with very short nanosecond pulses. It has been done by the help of Q-switched 1064-nm Nd:YAG lasers, with and without carbon suspension. Short pulses are used to target hair follicles resulting in rapid heating of the chromophore (melanin). It leads to photo acoustic shock waves that cause focal photomechanical disruption of the melanocytes but not complete follicular disruption. Therefore, the Q-switched Nd:YAG lasers are not likely to produce long-term hair removal.

A distinction needs to be made between permanent and complete hair loss. Immediately after laser treatment, the hair shaft shows fragmentation with focal rupture into the follicular epithelium and thermal damage to the surrounding follicular epithelium. The extent of thermal damage is dependent on the pulse width but retains confinement on the spatial scale of the follicle itself. Histological changes are almost guaranteed in the person undergoing laser hair removal. However, such form of hair removal should be carried out under the supervision of a qualified dermatologist as it involves considerable degree of expertise.

1. The ACTIVE or ANAGEN growth phase, consists of approximately 85 % of our body hair at any given moment in time and can last up to several years. During this stage, the hair shaft and follicle has an accumulation of pigment and is most affected by the laser light.

2. The REGRESSIVE or CATAGEN phase is the stage during which the hair stops growing but is not yet shed. This phase often lasts a few weeks comprising about 3 - 4% of our total body hair at any given moment in time. The laser will work in this stage, but if it is late in this part of the cycle, a new hair might already be coming through and the new hair will not be affected by the laser.

3. The RESTING or TELOGEN phase lasts 5 - 6 weeks, at the end of which the hair falls out and the new hair becomes visible. The new hair might not be affected by the laser because it will not have pigment in it yet, remembering that the laser actually relies on the pigment to find the hair. Approximately 10-13% of our body hair is in this phase at any one time.

Getting an appreciation of the hair growth cycle leads to the ultimate question, "Is it possible to get permanent hair removal?" The answer is Yes, Laser Hair Removal can certainly provide a level of permanence. As you can see, if a hair in the Active/Anagen stage is revealed to the laser, then that hair will most probably be killed permanently. If a hair that is in either of the other 2 stages is revealed to the laser, then that hair will most probably be killed, but the new hair coming through will still carry on growing.

Why Doesn’t Laser Hair Removal Always Permanently Remove Hair? (I)

Laser Hair Removal is now one of the most common ways to get rid of unwanted hair, and done right, can definitely provide permanent hair removal. But the capability of Laser Hair Removal is reliant upon numerous factors, including

(i) the variety of laser being used,
(ii) the color of your hair,
(iii) the color / variety of your skin and
(iv) the phase of the hair in the hair growth cycle.

Today there is at least 5 types of laser that are used for hair removal. Your skin type and you hair color will help to determine the most effective laser for you. And even if you get it all correct, you still might not be left with permanent hair removal in a short period of time. You see, the answer lies in the hair growth cycle, and more explicitly, what phase of the cycle your problem hair is in.... sounds baffling - it is !

Before we learn about the hair growth cycle, it is imperative to understand just how Laser Hair Removal really works. That is, to appreciate literally how you can accomplish permanent hair removal, acquiring an insight into the laser process is crucial.

The laser process doesn't actually physically remove the unwanted hair. The energy from the laser literally travels through the skin, and is drawn in by the pigment in the hair follicle and shaft. When this energy is drawn into a hair, the follicles are damaged, which then impairs that specific hairs capability to grow. Eventually the hair will fall out, usually within a matter of weeks.

Getting knowledge of how the laser operates is essential. You need to always remember that the laser doesn't just make the hair fall out on the spot. It actually impairs the capacity of a hair to grow, causing it to eventually fall out. Now that we have a brief understanding of how the laser performs, it is time to get an understanding of how the hair growth cycle affects the permanency of laser hair removal.
